Testimonial Permission Terms

I understand my testimonial provided (hereafter “Testimonial”) is made on behalf of Dovetail Contracting LLC (hereafter “Company”) and may be used in connection with Company’s marketing, advertising, and public relations.

 

I hereby authorize Company to copy, exhibit, publish or distribute my Testimonial, name, brief biographical information, and headshot photograph for purposes of publicizing Company’s services or for any other lawful purpose. These statements may be used in printed publications, multimedia presentations, on websites or social media, or in any other distribution or digital media. I agree that I will make no monetary or other claim against Company for the use of the Testimonial.

 

In addition, I waive any right to inspect or approve the published Testimonial, including written copy, wherein my name and likeness appears.

 

I hereby certify that I am over eighteen (18) years of age and competent to contract in my own name.

 

I understand that I have the right to revoke my Testimonial at any time by giving Company written notice of my revocation.

 

I hereby hold harmless and release Company from all claims, demands and causes of action which I, my heirs, representatives, executors, administrators or any other persons acting on my behalf or on behalf of my estate have or may have by reason of this authorization.
